Media-placeholder program batch 3: iMessage, Signal, and WhatsApp stop
minting <media:kind> placeholder bodies. Primary bodies carry captions
only with one aligned structured fact per native attachment (type-only
for missing/rejected/oversized). Echo, quote, and pending-history
records gain structured media facts so dedupe and previews work without
rendered text. Signal media-only outbound sends drop the placeholder
body entirely (signal-cli accepts attachment-only sends; recipients now
see just the media). WhatsApp audio preflight de-stringed; qa-lab
sticker recognizer moved to structured facts; WhatsApp gif maps to
video (presentation mode, no behavioral consumer).

* fix(agents): skip runtime plugin reload when a registry is already active
ensureRuntimePluginsLoaded rebuilt and activated a replacement plugin
registry on every call site invocation (agent turns, cron isolated-agent
runs, the gateway prewarm sidecar). Activating a replacement retires the
active registry and runs its plugin host cleanup, which cron.remove()s
persistent plugin-scheduled session turns before the replacement's async
re-registration catches up — silently deleting the cron row with only a
low-level 'cron service unavailable' WARN. Plugins using
api.session.workflow.scheduleSessionTurn for recurring jobs lose their
schedule on the next agent turn after boot.
Guard with getActiveRuntimePluginRegistry(): when a registry is already
active every caller's request is satisfied, so return before building a
replacement.
* fix(agents): make active-registry reuse compatibility-aware, not presence-only
The original guard (1de432a9) returned early whenever ANY registry was
active, without checking whether it was actually compatible with this
call's workspace, plugin scope, config, or runtime mode. A later call
requesting a different workspace could silently keep an unrelated
workspace's registry active instead of loading its own — a correctness
and workspace-isolation issue (caught in upstream review, PR #107752).
Fix: compute workspaceDir/startupPluginIds/loadOptions first, then check
getLoadedRuntimePluginRegistry({loadOptions, workspaceDir, requiredPluginIds})
before deciding to skip — reusing the existing loader cache-key
compatibility rules (workspace, config, runtime subagent mode, required
plugin IDs) that already govern normal cache-hit reuse elsewhere. Only
skips the reload when an active registry is genuinely compatible; still
loads a fresh one otherwise.
Tests: replaced the presence-only regression test with three cases —
compatible-registry reuse (skip), incompatible/different-workspace (still
loads), and startup-scoped plugin IDs passed through to the compatibility
check. All 5 acceptance criteria from the review verified locally:
runtime-plugins.test.ts, active-runtime-registry.test.ts,
standalone-runtime-registry-loader.test.ts, and the full cron lane
(33 files / 460 tests, including run.runtime-plugins.test.ts) all green;
pnpm build completes clean (5m14s, no errors).

The live codex text provider was a redundant projection of the openai
catalog (exclusive provider ownership; the openai plugin's ChatGPT OAuth
discovery already serves gpt-5.6-* route-aware). Folding it:
- extensions/codex no longer registers a text provider, catalog entry, or
synthetic text auth; provider.ts/provider-catalog.ts/provider-discovery.ts
and the route-blind model-name heuristics are deleted; the narrow
post-harness reasoning fallback moves to an app-server-owned module
- openai thinking policy keys on explicit selected-route provenance
(api === openai-chatgpt-responses) instead of value-shape inference
- models.list gains an optional additive agentRuntime field (configured
intent); session agentHarnessId remains the execution proof
- doctor --fix migrates the shipped codex/* config shape end to end:
every model slot, provider-config merge with blocker-aware conflict
handling, sessions, cron payloads (two-phase: runtime policy persists

Adds a Discord Activities integration so an agent can show a self-contained
HTML widget to Discord users, opened as a sandboxed Activity inside the client.
Off by default: routes, the discord_widget tool, and the launch handler register
only when channels.discord.activities is configured. OAuth identifies the user
and gates on the account allowlist; widget lookup is capability- or
instance-validated; token exchange is rate-limited; widget HTML runs in a
no-network sandboxed iframe.

on omitted sections; tailnet resolver returns a guaranteed string at
the source instead of a caller-side undefined throw.

PROVIDER_LABELS becomes a satisfies-typed closed record (static reads
provably defined; dynamic lookups go through providerUsageLabel with
honest string|undefined). Status-scan overview passes its optional

The original signature claimed ChatChannelMeta while leaking undefined
on bundled channel id metadata drift; three of four callers already
handled absence. The return type now says so, and the one assuming
caller falls back to the raw channel label.

Covers the sqlite-sessions flip and auth-source-plan code that landed
mid-phase, plus the channel-validation test consuming the now honestly
optional getChatChannelMeta.
* refactor(channels): split chat-meta accessors along the SDK contract
getChatChannelMeta keeps its shipped plugin-SDK signature (defined for
bundled ids, fail-loud on impossible misses); new findChatChannelMeta
carries the drift-tolerant optional contract for core auto-enable and

qa-channel spread getChatChannelMeta over an id that is never in the

expectDefined wrapped messages[i + 1] whose absence on the final message
is the normal case; the adjacent ternary already handled it. Restores
the plain optional read with an explicit guard in the pair condition.
* fix(plugin-sdk): channel plugin factory tolerates non-bundled channel ids again
createChannelPluginBase spreads bundled catalog meta for ANY channel id,
where absence is the normal case for external plugins; the resolver is
honestly optional again while the exported bundled-id accessor keeps the

Fresh-setup and first-install paths (crestodian setup inference, hook
installs, agent config base, target agent models) legitimately lack the
section being rebuilt; spreading undefined is the shipped {} semantics.
Removes the remaining gratuitous assertion wraps found by tree audit.
