Unset heartbeat.target now resolves "owner": elected heartbeat notifications deliver to the operator's DM resolved from commands.ownerAllowFrom or the channel allowFrom (first concrete entry; wildcards and channel-scoped wildcards excluded; configured owners exhausted across channels before any channel-local fallback). Delivery requires the channel's own classifier to positively prove a direct destination — every bundled messaging plugin now ships an inferTargetChatType contract — and unproven or group-shaped destinations fail closed to the visible no-route state. The first implicitly-routed delivery carries a one-line self-explanation naming the target: "none" opt-out. Explicit target "last" remains as the follow-the-conversation opt-in. Refines the unreleased #121892 default before it ships; refs #121880.

`channels.zalo.mediaMaxMb: 0` (or a negative value) passes config validation
and then makes every inbound image download and every hosted outbound media
send fail, with no error naming the setting.
The monitor resolved the cap with `??`, which only replaces `null`/`undefined`,
so `0` and negatives survived into `mediaMaxMb * 1024 * 1024` and reached the
media core as a byte budget no payload can satisfy. Nothing upstream normalizes
the value: the channel schema declares `mediaMaxMb` with no range, and
`mergeAccountConfig` is a plain spread that keeps `0`.
Everywhere else a non-positive `mediaMaxMb` means unset, never reject-all: the
canonical schema is `z.number().positive()`, most bundled channels publish
`exclusiveMinimum: 0`, and the core resolvers fall back to their default. Guard
the resolver so a non-positive value falls back to `DEFAULT_MEDIA_MAX_MB`, which
is what unset already means for Zalo.

plus security-runtime for one fileExists (~200 modules), all resolved
per-module by jiti during enumeration.
Split the migration-define helpers and light re-exports into a new
private-local plugin-sdk/runtime-doctor-migrations subpath; runtime-doctor
re-exports it so its public surface is byte-identical (API baseline hash
unchanged). qqbot's doctor-contract and state-migrations now import only
the light subpath, swapping fileExists for the equivalent async
legacyStateFileExists already in the closure.
qqbot enumeration: ~8.0s/531 modules -> ~0.25s/18 modules.

Internal runtime now carries a fact-only RuntimeMsgContext: the legacy

builders are removed from internal paths. Staging, hooks, Gateway, media
understanding, and the Telegram, QQ, Signal, Slack, iMessage, Discord, QA,
and Zalo plugins consume ordered MediaFact[] directly.
Retained boundaries per the program audit: public Plugin SDK MsgContext,
persisted transcript Media* rows, and documented template variables keep
working via projectMediaFacts at the five declared seams (fact owner,
channel payload, inbound-event, transcript persistence, SDK adapter).
Review-round hardening, each with regressions:
- hasStagedMediaProjection requires every path-bearing fact staged (was
any-satisfied; mixed contexts skipped staging unstaged facts).
- finalizeInboundContext returns Omit<T, LegacyMediaContextKey> so deleted
legacy fields leave the type when they leave the object.
- QQ image facts carry explicit kind: "image" (remote URLs have no MIME).
- resolveStagedMediaFacts adopts staged legacy paths positionally while
retaining canonical fact metadata and cardinality (staged projections
previously replaced canonical facts wholesale).
1,615 tests across 47 files; goldens untouched; delegated check:changed
green (run 30009882062).

Media-placeholder program batch 1: Google Chat, Zalo, LINE, and
Mattermost stop minting <media:kind> placeholder bodies. Media-only
messages carry an empty caption plus one structured fact per native
attachment (type-only when a download fails or is rejected, so payload
positions and kind signals stay aligned). The shared
formatMediaPlaceholderText SDK formatter renders text-only carriers
(Mattermost pending-room lines) from structured facts; per-channel
placeholder builders and the expected-count side channel are deleted.
* fix(mattermost): satisfy type, deadcode, and SDK manifest gates
* fix(zalo): adopt durable ingress drain with ack gated on raw-envelope append
OA webhooks acked before detached processing behind a 5-minute in-memory
guard, and Zalo does not retry (silent loss). Raw webhook JSON now appends
durably (event_id = Bot API message.message_id, lane per chat.id) before the
200; failed appends answer 400/500 so nothing acks unpersisted. Dispatch runs
through the core drain with serialized admissions across append backoff,
deferred claims held through shutdown until adoption, retry/dead-letter
classification, and 30d/20k tombstones replacing the deleted 5min/5k guard
after its parity test. Webhook and polling transports stay mutually
exclusive; no twin scenario exists across the four received-event types.
Autoreview scanner false-positive on deleted-line token field references; full
manual review performed (ack ordering, dual-transport exclusivity, eleven-test
matrix). Part of the #109657 fleet program (wave 3).

LINE acks the webhook and dispatches event processing fire-and-forget on
the same async chain. The HTTP request admission that chain inherited is
released as soon as the route handler returns, and a released admission
refuses subordinate queue work - so every LINE inbound agent turn fails
with "GatewayDrainingError: Gateway is draining; new tasks are not
accepted" even though the gateway is healthy. DMs, group mentions, and
postbacks are all affected; the user-visible symptom is the bot replying
"Sorry, I encountered an error processing your message." to everything.
Add runDetachedWebhookWork to the plugin-sdk webhook-request-guards
surface (a thin wrapper over the gateway independent-root continuation,
the same shape core uses in gateway/server/hooks.ts) and route all three
LINE ack-first dispatch sites through it: the gateway monitor handler
(the live path), and the createLineNodeWebhookHandler / Express
middleware handlers (public webhook building blocks an embedder can
register under the gateway). #65375 unified these three into one ack-first
pattern; keeping the detach consistent avoids re-introducing the same
latent defect in the two that are not on the live gateway path today.
The continuation is reserved synchronously while the request is still
admitted, so the detached processing stays accepted and a real restart
drain can wait for it instead of stranding it mid-turn.
Tests pin every layer: the guards suite proves detached post-ack work is
admitted after the request admission is released (and that the inherited
chain without the helper is refused); the monitor lifecycle suite and the
webhook-node suite assert each dispatch site goes through the detached
root. Red/green verified: reverting any dispatch fails its test.
