refactor(commitments): remove inferred follow-up subsystem

Remove hidden follow-up extraction, heartbeat delivery, CLI, docs, and supporting tests/tooling. Existing commitment records remain inert pending separately approved cleanup. Co-authored-by: Ayaan Zaidi <hi@obviy.us>
